Step #1: Reserve Equipment

Appointments are made by emailing audiovisual.education [at] mcgill.ca and must be made with a minimum of 3 working days in advance of the desired pick-up (date and time). All requests must include:

• • Desired date and time for equipment pick-up and return
  • Type of equipment to be borrowed
  • Checkout number (if available)

Appointment requests are not guaranteed. You must wait for a confirmation email to be assured the equipment and personnel are available to complete the transaction.

Step #2: Request Building Access

Fill out the form a minimum of 3 working days in advance of the equipment pickup/return date.

Step #3: Health Declaration

Fill out the 不良研究所 COVID-19 Self-Assessment Form on the day of the requested building access. To complete this process:

• Log onto Minerva
• Go to the 鈥淧ersonal鈥� tab
• Click on 鈥淐oronavirus (COVID-19) Self-Assessment for Students and Employees on Campus鈥� and fill out the form
• Submit the form to your immediate supervisor.
• Complete a duplicate copy of the form and submit to Degane.sougal [at] mcgill.ca. (i.e. You must complete the self-assessment twice, with two different recipients in the 鈥淪upervisor鈥檚 email鈥�; one copy to your immediate supervisor and one copy to Degane.sougal [at] mcgill.ca.)

不良研究所 requires each individual visiting the building to fill out this form prior to each time they enter a building. This process is monitored by the Faculty and the Emergency Operations Centre.

Step #4: Equipment Return

To gain building access to return the borrowed equipment, please repeat steps #2 and #3 as noted above, using the prescribed timeline (i.e. 3 days prior for building access form and day of for health declaration form).
