All听applicants applying to the MDCM or joint programs are required to submit proof of residency for their required applicant cohort (International, Canadian, or Quebec resident)听 听
狈辞迟别:听applicants to the Med-P Qualifying Year听are听not听required to submit such information unless specifically requested (e.g. presented in the application checklist in the online application system).
Residency for MDCM admission purposes is established as of the program application deadline. A change in residency after this date will not result in a change of applicant category. Residency documents are reviewed by the Undergraduate Medical 不良研究所 office, who will make a determination of Residency based on the validity of the documents submitted.听
Applicants whose documents do not fulfill residency requirements for the applicant category they have selected will be notified and switched to the appropriate category.
Note: Residency for tuition fee purposes is reviewed separately by Enrolment Services and is established based on the admitted student鈥檚 residency situation as of the start of classes.

Quebec Residents
Note that only Canadian citizens (including Canadian First Nations and Inuit) and Permanent Residents of Canada can claim Quebec residency.听 To be considered a Quebec resident as outlined by the Quebec government you must be able to prove your residency status according to one of the following 14 situations. 听
IMPORTANT听鈻 Quebec residency status must be听fully听established (not provisionally) by the application deadline of November 1st for all those applying to the MDCM program. Changes of citizenship or residency after the application deadline will not be considered.

You had Quebec residency status in university and are continuing on at another听university without interrupting your studies for more than two semesters (not including the summer semester).. 听Proof of Canadian citizenship or permanent residency听 If you are coming directly to 不良研究所 from another Quebec university where you were already a proven Quebec resident, we will normally not require any documentation from you because we can request听 that your information be transmitted to us directly by the Ministry of Education.听Note that this situation may not be used by Special or Visiting students for a Summer term. |

Situation 7You are receiving financial assistance from听听(AFE) Or you are the former recipient and did not interrupt your studies for more than two consecutive semesters (not including the summer semester). |

Situation 8You have lived three months in Quebec without having resided elsewhere in Canada for more than three months |
One of 3 scenarios must apply: 1)听听听听 Permanent Resident (landed immigrant) You are a permanent resident of Canada who has taken up residency in Quebec within the first听three months after your permanent resident landing date (i.e., the date you became a permanent resident). Provide: a.听听听听听 Proof of permanent residency.听听 AND: b.听听听听听 Proof of your Quebec residency since your arrival in the province. Either a copy of your valid听4 听issued Quebec听Medicare card听or听proof that your main domicile is in Quebec. OR, 2)听听听听 Returning Permanent Resident (landed immigrant) You are a permanent resident of Canada, who departs from Canada within the first three months of your date of landing and subsequently establish yourself in Quebec immediately upon your return to Canada. Provide: a.听听听听听 Proof of permanent residency. 听 AND: b.听听听听听 Proof of your activities outside of Canada for the period between when you first landed in Canada and when you returned to live in Quebec (e.g., enrolment letter from former school, academic transcripts clearly indicating range of dates of enrolment, proof of employment or residence). AND: c.听听听听听听 The听sworn听statement for returning permanent residents.听Note the statement must be signed by the commissioner of oaths for the purposes of Medicine Admissisons. AND: d.听听听听听 Proof of your Quebec residency since your arrival in the province: Either a copy of your valid听4听issued Quebec听Medicare card听or听proof that your main domicile is in Quebec. OR 3)听听听 Canadian citizen (Including Canadian citizens born abroad) 听You are a Canadian citizen living in Canada for the first time in your lifetime. Before entering Quebec, you never resided in another province or territory than Quebec听for more than three cumulative months. You have now been living in Quebec for at least three months. a.听听听听听 Either听a听Canadian birth certificate听[6]OR听Proof of Canadian citizenship. AND: b.听听听听听 Proof of activities for the 10-year period prior to your arrival in Quebec (e.g., enrolment letter from former school, academic transcripts clearly indicating dates of enrolment, proof of employment and/or residence) AND: c.听听听听听听 听The听sworn statement for Canadian citizens.听Note the statement must be signed by the commissioner of oaths for the purposes of Medicine Admissisons. AND: d.听听听听听 Proof of your Quebec residency since your arrival in the province. Either a copy of your validated听4 issued Quebec听Medicare card听or听proof that your main domicile is in Quebec. |
Situation 9Your spouse is considered to be a Quebec resident according to one of the 13 other Quebec residency situations |
Either a听Canadian birth certificate听[6]OR听proof of Canadian citizenship or permanent residency听OR听a听certificate of Indian status听OR听a听valid Makivik society card. AND, documents for 1 of these 3 scenarios: 1.听听听听 Legal Spouse by Marriage or Civil Union a.听 听 听 A copy of your marriage or civil union certificate. AND: b.听 听 听All of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec. OR, 2.听听听听 Common-law Spouse with child(ren) a. 听 听 听The听sworn statement听attesting to听one听year of cohabitation as proof of your common-law relationship with your spouse.听Note the statement must be signed by the commissioner of oaths for the purposes of Medicine Admissisons. AND: b. 听 听 A copy of the birth (or adoption) certificate for your or your spouse's child. It must听state听the names of both parents of the child. AND: c. 听 听 All of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec. OR, 3.听听听听 Common-law Spouse without child(ren) a. 听 听 听The听sworn statement听attesting to听three听years of cohabitation as proof of your common-law relationship with your spouse.听Note the statement must be signed by the commissioner of oaths for the purposes of Medicine Admissisons. AND: b. 听 听 All of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec. |
Situation 10You are a member of a First Nation or Inuit nation in Quebec Consult the Quebec Ministry of Education's list of听recognized nations |
Members of a First Nation in Quebec 1)听听听听听听 Your valid听Indian status card听issued by the federal government of Canada (both sides of card). Your card must indicate that you are a member of a recognized Quebec territory. If your card does not indicate a recognized Quebec territory, you must also provide one of the following: 2)听听听听听 Band council letter. A letter signed by your band council and on band letterhead, attesting that you are a member of a Ministry-recognized Quebec nation. OR, 3)听听听听听 James Bay Beneficiary card.听 If you are a James Bay Treaty beneficiary, a copy of your beneficiary card issued by the Cree Grand Council, as long as your beneficiary number is shown. Inuit beneficiaries of the James Bay and Northern Quebec Convention 1)听听听听听 Your valid听Makivik society card.听 If your card does not indicate a recognized Quebec territory, you must also provide: 2)听听听听 a letter from the Makivik Society confirming that you reside in Quebec |
Situation 11You continued to reside in Quebec even though your parents or sponsor no longer reside here. * You cannot have left听and then returned to Quebec since their departure. |
1)听听听听听 Either a听Canadian birth certificate听[6]OR听proof of Canadian citizenship or permanent residency听OR听a听certificate of Indian status听OR听a听valid Makivik society card. AND: 2)听听听听 A copy of a government-issued听document validating kinship between you and your parents OR听your Canadian听permanent resident landing paper which clearly indicates the name of your sponsor. AND: 3)听听听听 Proof that your principal and permanent domicile has been in Quebec since your parents or sponsor stopped residing in Quebec. 听Documents must be valid from the time your parent(s) or sponsor stopped residing in Quebec. 听听听Either a copy of your valid听4-issued听Quebec听Medicare card听or听proof that your main domicile is in Quebec. AND: 4)听听听听 Proof of Quebec residency听of one of your parents/sponsor up until their date of departure from the province. |
Situation 12You were听adopted by a person residing in Quebec at the time of your adoption. |

Situation 13Your two parents are deceased and one of your parents or your sponsor was residing in Quebec at the time of death |
Both of your parents must be deceased, but only one of the parents must have had听permanent domicile in Quebec at the time of听death. 1)听听听听听 Either a听Canadian birth certificate听[6]OR听proof of Canadian citizenship or permanent residency听OR听a听certificate of Indian status听OR听a听valid Makivik society card. AND: 2)听听听听 A copy of a government-issued听document validating kinship between you and your parents. AND: 3)听听听听 A copy of the听death certificate for each of your parents or sponsor (at least one of which was issued by the听Directeur de l'茅tat civil du Qu茅bec). |
Situation 14You have already been granted Quebec resident status in the past and in the past 5 years: 1)听You were enrolled at a Quebec post-secondary institution; and听 2)听Your tuition status at that Quebec post-secondary institution during your period of enrollment was that of a Quebec resident; and 鈥3) You were residing in Quebec for at least 3 consecutive years. |
Only select this situation if no other situation applies. Additional documentation may be required, and assessment may be delayed as this status must be validated by the Ministry of Education. 1)听听听听听听 Canadian birth certificate听[6]听OR听 Proof of Canadian citizenship or permanent residency OR Certificate of Indian status听翱搁 听Makivik society card AND: 2)听听听听听 Quebec听Medicare card听翱搁听proof that your main domicile is in Quebec; must be valid for听three consecutive years within the听five-year reference period immediately preceding the MDCM application deadline. |
[1]听Quebec birth certificate: Your Quebec birth certificate must have been issued by the听Directeur de l'etat civil, and must feature the notation听Certifi茅 conforme听or Certified. The certificate must have been issued after January 1994. Certificates marked听Certifi茅 conforme a l'article 137听are only acceptable to prove Canadian status, and not Quebec residency.听 Long-form Birth Certificates are preferred, but Short-form is acceptable if all necessary information for your selected Situation is included. If you were born before 1994 you can provide a Statement of live birth or birth registration certificate.听 Statement of live birth or birth registration certificates issued by the Quebec government must include the place of birth on the document in a clearly identifiable location in Quebec, e.g., Montreal. Statements of live birth that have been noted with the following are not acceptable: "This document is not a birth certificate" or "this document is not an act of birth".
[2]听Canadian Passport: A valid or expired Canadian passport is acceptable where the place of birth indicated is a clearly identifiable location in Quebec AND you already have a听Quebec permanent code听which has been听validated听to 不良研究所 by the Ministry of Education听(i.e. the perm code is visible in your Minerva account at Student>Student Accounts>View Tuition & Legal Status).
[3]听Parent/ sponsors:
a) Your parent/sponsor's听principal and permanent residence听must be in Quebec.
b) Your parent/sponsor is not required to reside in Quebec for a minimum time before you may benefit from Quebec tuition rates.
c) You are not required to reside with your parent/sponsor.
d) You do not have to be a minor.
[4]听RAMQ Card:听Quebec听Medicare card.听Expired cards are not acceptable.听
[5]听Full-time study听is defined as being registered for 12 or more credits in one semester, or for an activity which is considered equivalent to full-time status. (e.g. some internship or co-op programs, thesis full-time, thesis additional session, thesis evaluation, or non-thesis extension). Note that grades of W count towards your status as full-time.
[6]Note that a provincial "Statement of Live Birth" is not accepted, the Canadian Birth certificate is required. Contact your provincial Service office or Vital Statistics office to obtain this document.

The submission of false, incomplete, inconsistent or misleading information, or any omission that may result in a false or misleading conclusion, constitutes misconduct in the admission process. A finding of misconduct in the application process may lead to a refusal or, if an offer of admission has already been extended, a withdrawal of the offer at the sole discretion of the University.
