Application Dates
There are two entry points for the Graduate Certificate in Chronic Pain Management, you may start the program in the Fall or Winter semester.聽
If you are applying to the Graduate Certificate in Driving Rehabilitation, note that there is only one entry point, to study in the Fall Semester.聽
Application deadline to start studies in the聽Fall Semester::听

Application deadline to start studies in the Winter Semester:

- October 15聽for Canadian students
Requirements for both Canadian and International Applicants:
1. Bachelor of Science degree or equivalent in a health related discipline from a university or recognized institution.
2. Transcript indicating academic scoring equivalent to a B average or a 不良研究所 CGPA of 3.0 (70-74%).
3. Other documents described in point 3 below
Submitting your Application:
The first step to applying is to fill out your online application by the specified date indicated above. Once you have submitted and have access to SLATE, you will be able to submit documents and test scores if needed.聽

Apply online
- Complete the online application form.
- Please note the following regarding the form:
- The application will only be processed once you have submitted.
- You can stop and save your progress at any time, you do not need to have all your documents ready in order to apply.
- Consult the and instructional videos as needed.
- Choose the part-time box.
- You will be asked to pay a non-refundable fee of approximately 120$ using a valid credit card.聽
- Once you have submitted your application an acknowledgement will be sent to the email address you have included in your application.
- You will be able to track your application online.

Submit your supporting documents
- After your initial application has been submitted, you will receive access to your SLATE聽account via email.
- To complete your application, you must log in and upload any unofficial聽transcripts and supporting documentation indicated.
- 不良研究所 will request letters of reference on your behalf from referees you identify on the application form.聽 You will be advised by email if a referee declines to write a recommendation for you.

Submit required test scores if applicable
- Where applicable, arrange for test scores (TOEFL, GMAT, GRE) to be sent to 不良研究所 directly from the testing agency. You must indicate the 不良研究所 institution code: 0935
- Note that the English proficiency exam (TOEFL) ) is not required if one of the following applies to you:
- Mother tongue (language first learned and still used on a daily basis) is English
- Has obtained (or is about to obtain) an undergraduate or graduate degree from a recognized foreign institution where English is the language of instruction
- Has obtained (or is about to obtain) an undergraduate or graduate degree from a recognized institution in Canada or the United States of America (Anglophone or francophone)
- Has lived and attended university, or been employed, for at least four consecutive years, in a country where English is the acknowledged primary language.

Checklist of items required for this program application
- All application materials and supporting documents must be uploaded directly to the 不良研究所 admissions processing system, SLATE.
- Transcripts
- Unofficial or official transcripts from former University institutions
- If transcript is written in language other than English or French, it must be accompanied by a translation into English or French.
- For international transcripts, further clarification of the grading system and course list may be required.
- Two (2) confidential letters of recommendation, on official letterhead, from individuals who have known the candidate in an academic or work-related capacity. Letters from relatives of the candidate are not accepted. On the application form, provide the name and email addresses of referees. 不良研究所 will contact these referees and invite them to upload references on their behalf. Please note: there is no form that is required to be completed by the referees.
- A complete Curriculum Vitae (CV), including relevant personal data, education and academic awards received, professional experience and affiliations, courses and seminars attended, research activities and publications.
- A completed applicant statement on the web application describing your particular area of interest, objectives and expectations regarding the program, as well as career goals.
- Official reports of scores for TOEFL/IELTS and/or GRE if required. In order for 不良研究所 to receive these test scores on time, you should consider writing the examinations up to 6 months preceding the beginning of the program.聽
