The cost of the vaccine, if prescribed by a doctor, is covered by the 不良研究所 Supplemental Health Plan. The fee for the visit or administering听the injection is not covered.

Expense Policy:

Cost of vaccinations and/or medication required when traveling on University business and not covered by the University鈥檚 supplementary health plan will be reimbursed.

The original paper receipt or printed electronic invoice receipt is required.

If claiming the difference between the cost and the amount reimbursed by the University鈥檚 supplementary health plan, include the claim statement.

Registration Form for Faculty/Staff Traveling on University Business

All University business travel must be registered through this form to ensure coverage for any unforeseen medical emergencies while traveling outside your province of residence.

You must log in with your 不良研究所 credentials (email and password) to access this form.
